Registered Nurses
RN Case Managers create the overall plan of care to promote wellness and prevent re-hospitalization.
At 21HHS we offer opportunities for growth and increased responsibilities: RNI, RNII, RNII, RNIV
Licensed Vocational Nurses
Under direction of a RN Case Manager, LVNs assist and teach patient's and their family/caregiver with wound care, tube feeding, Foley, Congestive Heart Failure (CHF), medication and diabetes management among others.
Occupational
Therapy
The Occupational Therapist helps teach functional use cases to accomplish daily tasks. Primary goal is to find the right fit between patients abilities, need and desired activities and their home environment so patients can manage safely and productively at home.
Physical Therapist
Physical Therapist Case Managers assist with goal setting around core and strength training and conditioning, balance and mobility will prevent falls. .
At 21HHS we offer opportunities for growth and increased responsibilities: PT1, PTII
Physical Therapist Assistants
Under direction of a PT Case Manager, PTAs help patients who are recovering from injuries and illnesses to regain movement and manage pain.
Medical Social
Workers
Medical Social Workers assess the social, emotional and financial factors affecting patients and utilize community resources for a resolution.
